Graft Tunnel Mismatch in Anterior Cruciate Ligament Reconstruction with Bone-Patellar Tendon-Bone Grafts

Authors: Joshua R Eskew, MD, Daniel Miles, MD, Franky Davis, MD and Jeremy Bruce, MD

Abstract: Anterior cruciate ligament (ACL) reconstruction with bone-patellar tendon-bone (BTB) autograft is a very common surgical procedure for patients with ACL deficient knees; however, itcan be a technically challenging operation with possible intra-operative complications. Graft-tunnel mismatch (GTMM) is a condition in which the ACL BTB autograft does not match the intra-articular and tunnel length.

Salvage Procedure Salvaged by External Fixation: A Case Report

Authors: Ingrid Tuzi, Nader Chahidi and Konstantinos Drossos

Abstract: The one-bone forearm is a salvage procedure that can be used in chronic distal radio-ulnar joint instability, therefore sacrificing pronation-supination in order to reduce pain. Failed consolidation is a common complication and associated with several risk factors. Many authors insist on the need for rigid fixation, combining bone grafting, internal fixation and external fixation together when faced with complicated upper-limb reconstruction.

Intraoperative Fluoroscopy Improves Recreation of Femoral Offset but not Leg Lengths during Direct Anterior Total Hip Arthroplasty

Authors: Evan J Smith, MD, George Ibrahim, MD, Ryan D Scully, MD, Gautam Siram, MD, Anthony S Unger, MD and J Stuart Melvin, MD

Abstract: Postoperative leg length discrepancy (LLD) and increased femoral offset (FO) after total hip arthroplasty (THA) are frequently cited reasons for decreased patient satisfaction and litigation. Minimizing LLD and recreating FO can be challenging and different approaches exist.

Pyrocarbon Arthroplasty: An Innovative Alternative for Advanced Post-Traumatic Hamatometacarpal Arthritis

Authors: Quentin Baumann, Céline Klein, Emmanuel David, Nicolas Lebeau and Mirdad Moughabghab

Abstract: Post-traumatic arthritis of the hamatometacarpal complex (HMC) is a very rare disease and it occurs as an early secondary effect to undiagnosed or non-well treated fractures or fracture/dislocations of the 5th metacarpal basis. It frequently occurs to young patients and manual workers requiring an important strength and hand mobility.

Bilateral Ischiofemoral Impingement - Case Report and Surgical Technique of Endoscopic Treatment with Lesser Trochanter Partial Resection

Authors: Nuno Reais, Francisco Alpoim, Manuel Padin, Ricardo Frada, Jorge Cruz de Melo and Fernando Leal

Abstract: Ischiofemoral impingement (IFI) is a rare cause of hip pain. In this pathology there is a space conflict between ischial tuberosity and lesser trochanter, with damage to quadratus femoris muscle, causing hip pain of extra-articular origin. The best treatment choice for IFI remains unclear. The endoscopic treatment of IFI could be centered in the lesser trochanter or in the ischial tuberosity, and aims the decompression of IFS.

Increased Spinal Lumbar Fusion with Combined Magnetic Field (CMF) Bone Growth Stimulation

Authors: Rami Elsabeh, Rae Stegall and John Abrahams

Abstract: The aim of this study was to assess the efficacy of bone growth stimulation (BGS), specifically Combined Magnetic Field (CMF) stimulation, in the recovery of lumbar spinal fusion patients. Electrical BGS devices are prescribed postoperatively to help with the healing of spinal fusion patients by promoting fusion. This retrospective cohort analysis examined 316 patients who received lumbar spinal fusion surgery at Brain & Spine Surgeons of New York between the years 2013 and 2018.

Surgical Treatment of Neurofibromatosis-1 with Anterior-Posterior Fusion and Concave Strut Graft for Dystrophic Kyphoscoliosis with Dislocation: A Case Report with 10-Year Follow Up

Authors: Jane Lee, BCom, LLB, Noriaki Kawakami, MD, DMSc, Ryoji Tauchi, MD, DMSc, Kazuki Kawakami, BKin and Tetsuya Ohara, MD

Abstract: Although dystrophic Neurofibromatosis type-1 (NF-1) is associated with severe deformity, NF-1 with vertebral dislocation only is extremely rare. We present outcomes of a 10-year follow-up of a combined anterior-posterior fusion for the correction of severe dystrophic kyphoscoliosis with a T3/T4 dislocation in a pediatric patient with neurofibromatosis type 1.
