Haemodynamic Changes during Mandibular Third Molar Disimpaction

Authors: Moradeke Alonge, Oluwafemi Adewale Adesina, Taofiq Opaleye, Yussuf Salami, Idowu Omobolaji Stephen, Adebayo Kuburat Olayinka and Ademola Olaitan

Abstract: This study assesses the changes in the haemodynamic parameters of normotensive patients that underwent surgical extraction of impacted mandibular third molars following the administration of Lidocaine Hydrochloride (HCl) 2% with Adrenaline 1:80,000. This prospective cohort study was conducted on 168 patients who presented at the outpatient clinic with the indication(s) for surgical extraction. Patients aged 18 years and above, whose SBP was between 90 mmHg - 120 mmHg and whose DBP was between 60 mmHg - 80 mmHg, were included in the study. The patients' HP was taken at four different time points, namely T1, T2, T3, and T4, using a pre-calibrated non-invasive electronic BP machine (ANDON BPM) and Pulse oximeter (CA-MI).

Diazepam and Its Effects on Postoperative Sequelae of Surgically Extracted Impacted Mandibular Third Molars

Authors: Yussuf Salami, Oluwafemi Adewale Adesina, Taofiq Opaleye, John Chukwudumebi Wemambu and Ademola Olaitan

Abstract: This study evaluates the role of oral sedation, using 5 mg diazepam, on the postoperative sequelae (Pain, trismus and swelling) following impacted mandibular third molar surgery. This is a randomised control study involving 100 patients whose anxiety levels were assessed with the Modified Dental Anxiety Scale (MDAS) just before drug administration and thirty minutes after administering either Diazepam or placebo. The facial dimensions and maximum interincisal distance were measured and recorded preoperatively. After that, the surgical extraction of the impacted mandibular third molar was done. The Visual Analogue Scale was used to assess pain immediately after the procedure.

Removing Biofilms in the Isthmus by Endodontic Surgery

Authors: Xiuyou Wang and Li Peng

Abstract: Biofilms are multicellular communities which are composed of prokaryotic and/or eukaryotic cells embedded in matrix. More than 500 bacterial species have been identified in dental biofilms, which pose a harmful risk to dental health. Isthmus is a ribbon-shaped communication between two root canals which contains pulp tissue. It is almost impossible to be instrumented and cleaned completely in root canal treatment. It is not unusual to find a very narrow isthmus containing multi-layered microbial condensation and necrotic pulp tissues in the segment of the roots with periapical radiolucent lesions. Therefore, biofilm in isthmus cannot be neglected when we track the possible failure causes of root canal treatment.

A Hidden Oral Lesion

Authors: Sagar Khanna, BDS, DDS

Abstract: A 60-year-old man with a 10-pack-year smoking history was seen in the Oral surgery clinic for pain upon tongue movement especially with eating. A physical examination revealed a 1 cm by 3 cm exophytic mixed red and white lesion in the floor of the mouth and the ventral surface of the tongue extending posteriorly on the right side (Figure 1). On palpation, this was tender, firm, fixed, sessile with raised areas and rolled margins. Differential diagnoses included a squamous cell

Allogeneic Bone Graft for Stability of Inverted L Osteotomy Mandibular Ramus

Authors: Joao Luiz Carlini Eo Ms PhD, Tuanny Nascimento Eo Ms and Mauricio Romanovski Eo Ms

Abstract: In corrections of dent facial deformities where the mandible is the problem, the inverted 'L’ osteotomy may be a good option in specific cases, such as large mandibular advancements, condylar alterations, reoperations of the mandible, to increase the posterior facial height, among other situations. This technique was not much-used because the procedure used to be performed extra orally, leaving undesirable scars. We will describe the use of the technique via intraoral access, with allogeneic grafts and use of rigid internal fixation, exemplifying with some clinical cases.
