High Density Lipoprotein as a Biomarker, Potential Therapeutic Target and Therapy

Authors: Shirin Amini, DDS

Abstract: HDL is a plasma lipoprotein with the function of reverse cholesterol transport and inflammation modulation. Studies suggest that HDL can function as both anti inflammatory and proinflammatory depending on absence or presence of inflammation and chronic disease. Functions of HDL may not always match its cholesterol levels. LDL on the other hand is one of the main lipoproteins in plasma which is in charge of extracellular lipid transportation. With its major protein group being apolipoprotein B, LDL is capable of binding to extracellular matrix molecules within subendothelial spaces of arteries, causing a secretion of chemotactic agent, monocyte chemotactic protein 1 (MCP-1), which in turn triggers an inflammatory response of the type seen in atherosclerosis, all of these occur due to LDL lipid oxidation

Qtc Interval Dispersion in Patients with Acute Stroke

Authors: Francys E Guerra, Bárbara Carrera, Gabriel Schenone, Milángela Millán, Gabriela Díaz, Ana Febres

Abstract: QT, QTc interval and QTc dispersion express the inhomogeneity of left ventricular repolarization time. Its extension results in an increase of vulnerable period and susceptibility to malignant arrhythmias in patients with cardiovascular and cerebrovascular disease and hence sudden death.

Early Detect to Protect: Excess Epicardial Fat Volume is a Risk Marker for Cardiovascular Health Assessment

Authors: Mahfouz El Shahawy, MD, MS, FACP, FSCCT, FASH, FESC, FAHA, FACC, FASPC, Katelyn J Schultz, OMS IV, Sukanya Mohan, B.S., Genevieve M Palmer, OMS II, Frederick R Yturralde, MD, FACC, FSCAI and Antonella Sabatini, MD, P.E., M.S

Abstract: Standard modifiable or traditional cardiovascular risk factors, such as diabetes, hypertension, and dyslipidemia, have been implicated in enhancing the development of atherosclerotic cardiovascular disease. Recently, excess epicardial fat volume has been reported to be a cardiovascular risk marker. The objective of this study is to demonstrate that excess EFV is associated with structural and functional cardiac abnormalities, regardless of a subject's coronary artery calcium score, and emphasize the importance of epicardial fat volume quantification.

Influence of Concurrently-Existed Factors on Serum N-Terminal Pro-B-Type Natriuretic Peptide Level in Heart Failure

Authors: Mao-Sheng Huang, Ya-Ting Huang, I-Ling Chen, Min-Yu Lai, John Chen, and Chih-Chung Shiao

Abstract: The serum level of N-terminal pro-b-type natriuretic peptide (NT-pro BNP) would be affected by some other factors. To recognize the influential factors of serum NT-pro BNP level is very important for making accurate diagnosis and outcomeprediction. The study aimed to evaluate the influence of concurrently-existed factors on serum NT-pro BPN levels among patients with heart failure.

A Case of Complicated Prosthetic Valve Endocarditis Successfully Managed Conservatively

Authors: Sanjana Shah, MBBS and Abhisheik Prashar, MBBS, MMed

Abstract: Prosthetic valve endocarditis (PVE) is associated with high morbidity and carries a guarded overall prognosis. The management of this condition is based on consensus guidelines determined by various high-risk clinical and radiological features. Early surgery along with antibiotics has been the preferred option conventionally. We present a case of PVE with high-risk features that was successfully managed with antibiotics.

Assessment of the QT Interval in WPW: A New Approach to Calculating the QT Interval in Pre-excitation

Authors: Joshua W Kandiah, B.Sc and Simon W Rabkin, MD

Abstract: The initial slurring of the QRS complex, in WPW increases the QT interval rendering it a less accurate assessment of repolarization. The objective was to develop a method to calculate the underlying QT interval in WPW from the 12 lead ECG. Patients with a diagnosis of intermittent WPW on a single ECG had measurements of QT interval, delta wave duration, QRS duration and JT interval.

Preimplantation Genetic Testing for Inherited Predisposition to Cardiac Disease

Authors: Anver Kuliev, Tatiana Pakhalchuk, Maria Prokhorovich and Svetlana Rechitsky

Abstract: Cardiac disease, which is a common condition with genetic predisposition, has become a relatively frequent indication for preimplantation genetic testing (PGT), performed in the last dozen years to provide an option for couplers at risk to avoid the birth of an offspring without predisposition to cardiac disease. We present here our experience of 109 PGT cycles for cardiac disease, resulting in birth of 54 mutation free children
