Association of Hyperhomocysteinemia and Adverse Postoperative Outcomes of on-Pump Primary Isolated Coronary Artery Bypass Grafting Surgery among Postmenopausal Women

Authors: Rafael Kmiliauskis Santos Gomes, PhD and Moacyr Roberto Cuce Nobre, PhD

Abstract: Studies investigating the possible role of hyperhomocysteinemia (HHC) in increasing postoperative morbidity and mortality among postmenopausal women undergoing on-pump primary isolated coronary artery bypass grafting (CABG) surgery are sparse in the literature. The knowledge about the relationship of preoperative HHC and postoperative adverse cardiovascular events is not adequately covered.

The Impact of Ischemic Heart Disease on Rheumatoid Arthritis

Authors: Rafael Kmiliauskis Santos Gomes, PhD and Moacyr Roberto Cuce Nobre, PhD

Abstract: Rheumatoid arthritis (RA) is a systemic inflammatory autoimmune disease of unknown etiology. Current knowledge supports that chronic inflammation represents a crucial risk factor without prior development of atherosclerosis. Thus, the epidemiological findings of patients with RA have demonstrated an expressive increase in cardiovascular morbidity and mortality. In addition, these patients present a higher prevalence of coronary risk factors and, when associated with poor prognostic factors, increase the risk of ischemic heart disease.

Second Level Management of a Complex Case with Suspected Cardiac Amyloidosis: A Challenge for the Clinician

Authors: Valerio Massimo Magro

Abstract: We report the clinical case report of a 67-year-old patient with diagnoses of hypertension, ischemic heart disease and a previous history of Acute Myocardial Infarction (AMI) and nonspecific signs and symptoms (general illness, weight loss, hyporexia, fever, impressive edema at the feet, systemic arterial hypotension). So, we performed a series of laboratory, radiological and instrumental examinations, but the clinical picture remained difficult to interpret. The echocardiographic evaluation led us to a suspicion of infiltrative heart disease, and the magnetic resonance, while confirming the interesting data observed at the ultrasound exam.

Survival of Out-of-Hospital Cardiac Arrest by Early Defibrillation in the Sorrento Peninsula

Authors: Maurizio Santomauro, Vincenzo Iaccarino, Salvatore Criscuolo, Adele Ferro, Carla Riganti, Mario Alberto Santomauro, Raffaele Giordano, Mario Petretta, Francesco Elia and Domenico Bonaduce

Abstract: Out-of-hospital cardiac arrest (OHCA) is a critical public health problem also in Italy. The use of Automated External Defibrillator (AED) by non-healthcare professionals was approved in Italy since 2001. The number of installed AED was increased in Italy annually since 1999 and the number of sold AED was more than 50.000 units until 2017. Nevertheless, there were about 60.000 OHCA annually. For this reason, we investigated, using retrospective regional register study, the efficacy of Public Access Defibrillation (PAD) interventional program in terms of survival rate for patient suffering of OHCA with shockable rhythm rescued by bystanders compared to traditional 118 Emergency Medical Service (EMS) System activation.
